before your appointment
To help us provide the most accurate and thorough assessment, please bring along:
-
Any recent medical imaging you may have (such as X-rays, CT or MRI scans, or reports). If you’re unable to access these, don’t worry — your appointment can still go ahead.
-
A valid photo ID.
-
You’ll also be asked to complete and sign a consent form before your assessment begins.
What to wear: We recommend loose, comfortable clothing that allows the assessor to examine the affected area with ease.
during your appointment
Your consultation will usually include three parts unless stated otherwise:
-
Discussion – Our assessor will ask about your health history, how the injury or condition occurred, how it is currently affecting you.
-
History- The assessor may also ask you about previous employment, education, qualifications and social history.
-
Examination – A physical assessment may be completed to help inform the report including a combination of questionnaires. (not required for psychological or psychiatric evaluations).

Need to reschedule? If your appointment time doesn’t suit, you may be able to change it by contacting the organisation that arranged the referral.
after your appointment
Following your appointment, the assessor will prepare a detailed independent report.
This report is based on:
-
Your clinical history,
-
The findings of the assessment, and
-
Any documentation provided beforehand.

The completed report is sent directly to the organisation that referred you (e.g., your lawyer, insurer, or case manager). It is not provided directly to you.
